Info – Change of Details Please provide your old and new details for things such as your title, name, mobile number. We may ask for proof in response. If you have moved, please give the names of everyone in your household who this applies to, along with your old and new addresses. Please note that if you have moved out of the practice area, you will be given 30 days to move to another local surgery before being deducted from us.

Info – Appointments If you would like to confirm or cancel your appointment, please let us know your name and date of birth, and provide a contact number as well.

Info – Prescriptions Please give us as much information regarding medication, doses, and quantities, and include your name, date of birth, and contact number.

Info – Fit Note Please note that you can self-certify if you have been off for under a week. You may first need to consult with a clinician if you are requesting a new note. If you would like to renew, please confirm your name, date of birth, contact number, and the date from when you would like the note and the duration. Please also confirm whether you would like to collect the note from the surgery or if you would like us to text or email it.

Info – Medical Records You may request a copy of your medical records. An electronic copy can be provided within 30 days of your request. We can also enable you to see your records via online services. Please note we do not routinely print copies of medical records.

Info – Online Services If you would like to get set up for online services, or would like proxy access, or access to your full medical records, please use this form. We can also help troubleshoot any issues. Please ensure you include a contact number as well.

Info – Other You may use this to submit information that has been requested by us, or for any other enquiries.
