New Patients

New Patient Registration

All patients within the practice boundary are welcome to apply to join our practice list. You will be asked to complete registration and medical questionnaire form. Once you have completed the forms, please make an appointment for your new patient health check with the Practice Nurse.

Unfortunately, we are not always able to accept patients living outside of this defined area. All current patients who move out of our practice area, unless discussed with their registered GP, may be expected to change practices. If you have difficulty or need assistance in locating other primary care medical services you can contact the North East Primary Care Services Agency.

As a patient you also have the right to express a preference to which GP you see. As a practice we will endeavour to comply with your request, however this may not be possible at all times.

When registering, it is useful (although not currently essential) if you have your NHS number. This number is unique to you and can help find your records more easily and also helps to improve safety between health agencies involved in your care.

Medical treatment is available from the date of registration. Please contact reception for further information.

The Royal College of General Practitioners has produced a useful guide for patients about the services on offer at GP Surgeries and how to access them.

Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
