Community Outreach and Cultural Sensitivity at the Heart of Care

At The Grange Medical Practice, we believe healthcare should be accessible to everyone, regardless of background. We’ve seen firsthand how some communities face barriers to getting the care they deserve, and have made it our mission to address these inequalities through dedicated outreach by hosting events at local community centres. These events help us raise awareness about early screening and its importance in diagnosing crucial conditions like diabetes and cancer early on. Through these efforts, we hope to make a difference in the lives of those who need it most.

Our Health Inequalities Lead, Dr. Ilyas Ahmad, says,

“These events allow us to understand the unique needs of our patients. It’s about much more than medical care – it’s about listening and empowering people to take charge of their health.”

With the help of our Designated Care Coordinators, we’re connecting patients to the right services and support, ensuring no one is left behind irrespective of the cultural and communication barriers.

Partnering with NHS ICB, Kirklees Council and community healthcare leaders, we’ve opened new doors to conversations around health, tackling misconceptions, and ensuring everyone feel heard.

We are proud to see improvements, especially among our South Asian patients, and we’re committed to continuing this work. Addressing health inequalities isn’t just about healthcare – it’s about community, understanding, and partnership.
