New Patients

How to Register

If you live in our practice area and would like to register with us, please complete one of our registration forms that is available from reception.

All patients will have a named accountable GP.

Alternatively you can download and print off the 2 forms below:

  1. GMS1 Family Doctor Services Registration Form
  2. New Patient Registration Information Card

Please fill these in and hand them in at the Reception at either Batley Cherry Tree Surgery or Gomersal Surgery.

Non-urgent advice: Patient Notice

We cannot accept New Registration Forms by email, please bring them to the Surgery in person.

Practice Boundary

This practice is within the North Kirklees CCG area.

Please ensure you live within our service boundary before attempting to complete a registration form.
