Linking Leeds is the integrated city-wide Social Prescribing service for people in Leeds. We link individuals with a range of local community services to improve social, emotional and mental wellbeing. We accept referrals from GP surgeries, other professionals and self-referrals for everyone ages 16 years and above, regardless of where they live, as long as they are registered with a Leeds GP.
