Looking to Register With us?
Here’s what you need to know before registering:
- Patients are welcome to register at The Grange Medical Centre if they have a Leeds 14 postal code address and live within the Seacroft/Whinmoor areas. Patients who have a Leeds 15 postal code address (Cross Gates, Colton or Whikirk areas) and also people living in Swillington, can register at Colton Mill Medical Centre.
- When registering at the Practice patients will be required to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for patients medical records to arrive from their previous GP Surgery.
- We also ask that new patients obtain their NHS number from their previous GP surgery for ease of registration.
- New patients are able to collect our registration forms from our reception team, alternatively, patients may wish to print the registration forms at home using the links below.
- If patients wish to sign up for our Online Service at the time of registering, we require identification and proof of address.
- We would encourage patients who take regular medications to obtain a prescription from their current Practice before registering; this will ensure that patients have enough medication to last them until they are able to attend a New Patient Review Appointment at our Practice.
- All new patients are offered a new patient health check with a member of our Healthcare Team.
- Medical treatment is available from the date of registration. Please contact our reception team for further information.
New Patient Registration Forms
New patients are able to collect our registration forms from the reception team, alternatively patients may wish to print and complete the regsiteration forms at home using the links below:

Once you have completed the registration forms, please leave them with a member of our reception team at one of our sites or email them to colton.andthegrange@nhs.net.
Temporary Patient Registrations
If you are ill while away from home or if you are not registered with a doctor but need to see one, you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
