Care Navigation
Care Navigators are usually the first point of contact for a patient contacting the surgery. They support patients by directing them to the right healthcare provider to help with their problem. They also liaise with the wider practice team to manage patient queries.
GP’s
GPs provide a wide range of medical services including diagnosis, treatment, and preventative care for a broad range of health issues. They manage acute & long-term illnesses, mental health problems & palliative care, provide health education, and preventative screenings, and serve as a gateway to specialist services when referrals are needed. They supervise & provide guidance to other clinicians within the wider practice team.
Practice Nurses
Practice nurses work alongside GPs to provide a range of clinical services. They perform immunisations, wound care, blood pressure checks, managing long-term conditions (e.g., diabetes, asthma), & conducting health screenings. Practice nurses play a critical role in preventive care & patient education, helping patients manage long-term illnesses & encouraging healthier lifestyles. Some of our practice nurses are independent prescribers.
Advanced Clinical Practitioner
ACPs are highly skilled clinicians such as nurses and paramedics who have undertaken extra training to be able to help patients with more complex problems. They can diagnose, prescribe medication, and manage patient care similar to GPs, often dealing with minor ailments, long-term conditions, and urgent care issues.
Clinical Pharmacists
Clinical pharmacists in GP practices manage medication-related needs, conduct medication reviews, & provide advice on prescriptions. They work closely with patients, especially those on multiple medications, to ensure safe & effective drug use. Pharmacists also support management of long-term conditions like high blood pressure & diabetes & are key in delivering personalised medication advice & health education.
Pharmacy Technicians
Pharmacy Technicians are involved in medicines reconciliation, particularly for patients transitioning between care settings (e.g., from hospital to home). They help with repeat prescriptions and ensure that patients have an accurate list of medications and updating records to ensure safe continuity of care. They also support the monitoring and review of prescriptions, helping to detect and prevent potential adverse drug interactions.
Health Care Assistants
HCAs support clinical staff by performing tasks such as blood pressure checks and ECGs, and assisting with health checks and procedures such as coil fittings. They play an essential role in supporting preventive care, such as by helping patients with lifestyle advice and assisting with weight management.
Social Prescribers
Social prescribers connect patients affected by issues such as loneliness, mental health issues and social isolation with community resources to improve their health. They work with patients to create tailored plans that might include access to exercise programs, social groups, or financial advice, addressing non-medical factors that can impact overall well-being.
Mental Health Practitioners
Mental health practitioners at the surgery provide psychological support for patients with mild to moderate mental health conditions. They can give advice on managing mental health challenges & play a vital role in the early detection & intervention of mental health issues.
First-contact Physiotherapists
First-contact physios assess, diagnose, and treat musculoskeletal issues, instead of needing to see a GP first. They provide exercises, lifestyle advice, & physical therapies that can improve mobility & manage pain, supporting patients with conditions like arthritis, back pain & sports injuries.
Administration Team
The practice administration team manage patient records and other administrative tasks that help the practice run smoothly. These include letters, reports and referrals.
Care Co-ordinators
Care coordinators assist in coordinating between various healthcare and community and care services, ensuring patients receive comprehensive, coordinated care. They work with patients and families to develop personalized care plans and support them in managing long-term conditions.
GP Assistant
GP Assistants provide a support role, carrying out administrative tasks, combined with basic clinical duties such as blood pressure checks, ECGs and dressings. They also perform health checks & can give basic advice to patients about tests performed at the practice (such as spirometry) & lifestyle advice.
Practice Management Team
The practice manager and their deputy are responsible for the administrative, financial, and operational functions of the practice. They ensure that the practice runs smoothly and that both patients and staff have a positive, efficient experience.
