CHD reviews are carried out by our Nursing Team. You will receive an invite for your annual review when this is due.

COPD reviews are carried out by our Nursing Team, primarily by telephone. For patients who have less well controlled COPD, we will arrange for a face-to-face review with a member of the Nursing Team.

We provide Asthma reviews to all patients on the Asthma Register.

If your Asthma is well controlled, your annual review may be done by telephone or even a questionnaire sent via text message.

If your Asthma is not well controlled, we will arrange for you to see one of our Practice Nurses face-to-face.
