Catchment Area

You will be assigned to a “usual GP” when you join the practice however, you may consult with any of the GPs at the Practice.

    Complete our GSM1 Registration Form & New Patient Questionnaire
  • In Person
    Visit us at Reception, ask for our paper Registration Forms

Registering for Online Services

After you are fully registered with us you may request online access to allow you to book appointments, order repeat medication and view certain aspects of your medical record.

We require photo ID for this if you have not already provided it.

Non-urgent advice: Please note

If you are on any routine medication you will be offered a registration appointment with the doctor. The nurses offer new patient checks to those who are not on any medication.

If you move out of the area while you are registered with us you should register with another GP in whose area you live. 

Disabled Patient Facilities

The practice has a car park with limited space, and is about two minutes walk from the nearest bus stop. 

Our health centre has suitable access for disabled patients.

All the patient areas including the waiting room, the consulting rooms and the toilets, have wheelchair access. If you require one, there is a wheelchair available at reception.

There are designated disabled parking spaces in the car park.

There is  hearing loop in our reception area.
