New Patients

New Patient Registration

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. You need to live in the following areas:

• Ruddington
• Bunny and Bradmore
• Edwalton
• Silverdale
• Wilford
• Compton Acres
• West Bridgford – please check with reception as not all of West Bridgford is included
• Clifton – We are not currently taking new patients from Clifton

If you wish to register with us, we ask that you complete the following – Registration Form, New Patient Health Questionnaire which includes our policy consent for Sharing your GP record form and optional online access services, which will provide us with useful information whilst we wait for your medical records to arrive from your previous surgery. All forms can be picked up from the surgery in one of our new patient packs or they can be downloaded from below.

If you require information or communication in an alternative format (eg. large print, braille) please indicate this on your Health Questionnaire. We will then make reasonable adjustments to accommodate your needs. In addition, the following website has some useful information- including auditory and visual aids.

Medical treatment is available from the date of registration. Please contact reception for further information.

If you have repeat medication we advise that you request a month’s supply at your old practice BEFORE you submit your New Patient Registration pack to this practice. You may ask them to send the prescription to your new area pharmacy for collection (maybe a pharmacy near your postcode).
