New Patient Registration

To be registered with Southwell Medical Centre you must permanently reside within the practice catchment area shown on the map opposite or be a registered student at Brackenhurst. 

If you are in our catchment area and would like to register, please collect registration forms from reception. It is important to complete both parts of the form. If these are not both completed in full, it may delay your registration.

Students, please provide a photo of your photo ID such as your student card or driver’s licence. This can be emailed to nnicb-nn.c84049@nhs.net

Please note routinely it may take up to 7 Days to process your registration documents once you have brought them to the practice.

Finding your NHS Number please use the link below to find your NHS number this is important to find when applying for our surgery . This makes things a lot easier to find you on the system when getting you registered at the practice . This can also help us to link your records from your old GP practice . We would advise that you make a note of this Number as you may need it for NHS app and for when booking an appointment online with us . NHS number can be used when using Systm Connect to help us find you as a patient and to be able to link you to your records when submitting a request .
