If you’ve been off sick for 7 days or less
If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. We call this self-certification.

Referrals, Letters and Reports
When you and your GP agree that you need an appointment with a specialist, the E-Referrals System shows your GP which hospitals or clinics are available for your treatment. Your GP will discuss with you the appropriate options which are available for treating your medical condition. If you know where and when you would like to be seen, you can book your appointment before you leave the surgery.
You will be given confirmation of the place, date and time of your appointment. You may want more time to consider your choices. If so, you can take the appointment request letter away with you and book your appointment later.

If you need to go to hospital to see a specialist, you have the right to choose which hospital you’re referred to by your GP. This legal right, which was introduced in April 2009, lets you choose from any hospital offering a suitable treatment that meets NHS standards and costs. Medical Reports
1. Request Submission
Required Information: The request should include the patient’s full name, date of birth, and specific details about the information required in the report.
Written Requests: All requests for medical reports must be submitted in writing.
- 28-Day Turnaround: The practice aims to complete all medical reports within 28 days of receiving the written request. This timeframe allows for thorough review and accurate completion of the report.
- Clinical Review: A designated GP will review the patient’s medical records and compile the necessary information for the report. This may involve consultations with other healthcare professionals if additional information is required.
Text Message Notification: Once the medical report is complete, the practice will send a text message to the patient (or the requesting party, if appropriate) to inform them that the report is ready for collection or has been sent to the specified recipient, charges may apply.
