Register With Our Practice

Our practice is offering online registration. This is open to all residents withthin our catchment area.

Please note that if you want to come in person to register, we will be happy to help you. Please check that your postcode is within our catchment area and telephone the practice for further help. You do not have to show any ID or proof of address to register.

To register in person please bring your medical card or NHS number and details of your previous GP. You have the choice to see any of the GPs at the practice. If you do not know your NHS number please visit ‘visit the find your NHS number service’

All newly registered patients will be asked to complete a lifestyle questionnaire and maybe requested to book an appointment with a health professional for a health assessment within one month of registering. The health assessment also gives us the opportunity to meet you and your family and for us to tell you about the practice.

There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area. You can find a GP in your area by visiting ‘Find a GP in your area’.

We are keen to meet our new patients so that we can try and meet your requirements. We therefore ask you to make an appointment with a practice nurse when you register. Patients with complex medical problems or currently taking medication should first consult with their new GP.

Your registration will be responded to within 2 working days.Please complete one form for each family member.

If you wish to start your registration process by completing the relevant forms beforehand, you can print them off from the links below and bring them to the surgery.

Please note routinely it may take up to 48 hours to process your registration documents once you have brought them to the practice.
