How to Register
To register with our practice you can come into the surgery and complete a registration form, or you can complete our online registration process.
We may also ask that you also complete a new patient health questionnaire at the time of registration. Your medical records often take time to arrive from your previous doctor and this gives us the opportunity to record basic information about you and offer any immediate care you may need.
If you require a prescription urgently please contact us to advise the receptionist as you may need an appointment with a doctor, or our pharmacist. It may take up to two weeks to register you. Once registered you can phone to book an appointment. Our telephone lines open at 8am for urgent appointments. For a routine appointment you can telephone the surgery at any time during opening hours and our receptionists will be happy to assist you.
We do not exclude patients from the Practice on grounds of race, sexuality, religion, social class, age, disability or medical condition.
Proof of Identity
You are asked to bring with you one proof of identity that includes a photograph and one that includes your current address. We can still register you without these types of documents, at the Practice Manager’s discretion, but we prefer not to in order to help prevent fraud.
If you have a medical card please remember to bring it with you as this helps in transferring records from your current GP.
Below is a list of documents that are acceptable to help establish identity:
- Birth Certificate
- Marriage certificate
- Medical card
- Driving license
- Passport
- Local authority rent card
- Paid utility bill
- Bank/building society card statement
- National insurance number card
- Payslip
- Letter from benefits Agency/Benefits book/signing on card
- Papers from the Home Office
- P45
You will be registered with the Practice rather than a specific doctor, but you do have the right to ask to see a particular doctor if you wish.
