We are now using a new online service called Register with a GP Surgery that makes it easy to register with North Leverton Surgery.

The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.
Paper forms are still available if you need one. Please see below PRF1 Form which you can download, print and complete.
We ask that you complete as much of this registration form as possible in order for us to offer you the highest standard of care.

- Please note you must be living within our boundary to register with us.
- Patients taking regular medications will need a new patient health check with a doctor.
- Patients not taking regular medications will need a new patient health check with a nurse
