New Patient Registration

If you wish to register as a new patient with the practice, please check at reception that you live within the practice boundary area by entering your post code below:

Catchment Area

New patient registrations are only accepted subject to seeing 2 forms of Identity to verify your name and your address. Please ask reception for more details.

Forms of identity needed to prove name and address

  • Proof of Current Address i.e.Utility Bill or Bank Statement
  • Photo Identification i.e. Passport or Drivers Liscence

All new patients are asked to fill out a registration form and must attend a new patient health check appointment with the healthcare assistant. You will not be registered until you have attended, but may be seen by a doctor as a temporary resident if the doctor considers it to be urgent and necessary.

Non-urgent advice: Patient Notice

Note that by emailing the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.

Alternatively you may print off a registration form, fill it out and either post or bring it to the practice.

If you move house to outside of the practice Shiregreen Map boundary area, we are required to ask you to leave our surgery to register with another surgery immediately.
