Important Information
All requests received before 12pm will be reviewed on the same working day by our duty team; a normal appointment lasts 10 minutes, so one problem per appointment please.
If you are seriously unwell, call 111 or 999.
- The submitted forms will only be read during office hours.
- The forms will not be read on the weekends (Saturdays and Sundays) or on bank holidays.
- They will not be read Monday-Friday between the hours of 1200 – 0700.
