Pharmabox Registration/Sign-up

Following feedback from our patients the application form for use of the Pharmabox, 24 hour prescription collection point is now available to download, see document below.

Please download a copy and once fully completed return to Mattishall Pharmacy, 15 Dereham Road, Mattishall,  NR20 3QA or email to mattishalldispensary.d82039@nhs.net

The Pharmabox is not suitable for all patients and certain criteria may exclude you from using this facility, before completing an application form please review the below criteria.

Does your prescription contain any of the following: 

1. Glass/large bottles such as Gaviscon, Peptac , lactulose

2. Large heavy Creams such as Cetraben, Dermol, Zerobase,

3. Large boxes such as Macrogol, cosmocol, baby milk.

4. Large NWOS items  (dressings, catheters etc.)

5. Supplement drinks 

6. Refrigerated Items including inhalers 

7. Controlled drugs such as Pregabalin, Gabapentin etc. Diazepam/Zopiclone,  

8. Any accountable controlled drug

9. On our Free Repeat Prescription Service (FRPS) (at the moment we are unable to put these in as you would be unable to reorder at the point of collection)

10. Has 6 items or more (this would need to be checked due to the size of the items and how many of each)   

If any of the above are relevant to you, then unfortunately you are unable to have your medications dispensed via the Pharmabox 24 hour Prescription Collection Point

If you have none of the above and you wish to use this service please complete the downloadable consent form and return to us. 

Note it is essential you provide your correct mobile telephone number as we will be required to send you an SMS text message containing your unique PIN, each time you have a prescription for collection.
