From 6th May 2025, we will be introducing a new โtotal triageโ system for booking all appointments at our practice. This means:
โ๏ธAll appointment requests and queries will be directed through our new online request form
โ๏ธ A GP will review your request within hours and decide on the most appropriate way to meet your needs
โ๏ธOur reception team will assist those without online access by completing the form over the phone or in person

๐น Call us, and we can send you a link to access the service.
๐น If you have problems accessing this form, visit the surgery โ we will either fill in the form for you or send you a link and help you complete the form in person.
๐ What We Expect from You
โ๏ธ Provide the correct contact number on the form
โ๏ธ Once triaged, we will try to call you twice โ please ensure youโre available
โ๏ธ If we are unable to reach you after two attempts, a new online form will need to be submitted
Please note: There are certain conditions for which you do not need to fill in an online consultation form to see a clinician. You can self-refer to your local pharmacy.
