🏥 Changes Coming to Wheatfield Surgery – From 6th May 2025

Dear Patients,

From 6th May 2025, we will be introducing a new ‘total triage’ system for booking all appointments at our practice. This means:

✔️All appointment requests and queries will be directed through our new online request form
✔️ A GP will review your request within hours and decide on the most appropriate way to meet your needs
✔️Our reception team will assist those without online access by completing the form over the phone or in person

🔹 If you have problems accessing this form, visit the surgery – we will either fill in the form for you or send you a link and help you complete the form in person.

📋 What We Expect from You

✔️ Provide the correct contact number on the form
✔️ Once triaged, we will try to call you twice – please ensure you’re available
✔️ If we are unable to reach you after two attempts, a new online form will need to be submitted

Please note: There are certain conditions for which you do not need to fill in an online consultation form to see a clinician. You can self-refer to your local pharmacy.
