If you’ve been off sick for more than 7 days
You can obtain sick notes from the practice. They are for absence from work only.
If you are absent from work for more than 1 week you will need a doctor’s sick note to cover the period. If it is your first sick note for the current illness, then you will need to be seen by the first day that the note is required as the first note can not be back dated. When we see you, we will assess your condition and see if a sick note is required.
We are unable to provide you with a first sick note if we haven’t assessed you and any sick note given can only be valid from the date you were assessed.

If you’ve been off sick for 7 days or less
If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. We call this self-certification.
