We offer an NHS Health Check every 5 years, once you are eligible we will invite you either by SMS, telephone or a letter
The NHS Health Check is a health check-up for adults in England aged 40 to 74. It’s designed to spot early signs of stroke, kidney disease, heart disease, type 2 diabetes or dementia. As we get older, we have a higher risk of developing one of these conditions. An NHS Health Check helps find ways to lower this risk.

NHS Health Check
To request a copy of the vaccinations you have recorded on your medical record, please submit a routine request via the triage form. Please note, it may take up to 7 working days to provide this information to you.
Please visit the NHS recommended Travel Health information website Travel Health Pro to find out about vaccinations you may require, depending on your destination.
NHS GP practices only provide the following vaccinations:
- Cholera
- Hepatitis A
- Polio
- Typhoid
Please check your vaccination history and types of vaccinations required before requesting an appointment.
If you require other vaccinations not listed above, you will need to arrange to have these done at a private vaccination clinic – the major, national pharmacy chains (Boots, Superdrug etc) offer travel vaccinations, and you can search for clinics local to you online.
Please ensure you follow guidance on Travel Health Pro and allow adequate time before you travel.
