To request a sick note please complete the e consultation form Consult Online from Home – Rosemary Medical Centre and select admin option.
If you are unable to complete this form online you can phone the surgery on 020 8346 1997 and we can complete this form on your behalf. We have AI assistance that will complete the form for you and this can also translate from a number of languages.

If you’ve been off sick for 7 days or less
If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. We call this self-certification.
