Not feeling like reaching out to your GP? No problem! The Self-Referral service lets you or your family members refer you directly.

What happens after your GP refers you?

1. The referral is sent:

  • Your doctor will send a referral to the service, which includes information about your needs and why they think you would benefit from their services.
  • The referral service will receive the referral and review it to make sure it has all the necessary information.

2. The referral is processed:

  • The referral service will contact you to let you know they have received the referral and to ask any questions they may have.
  • They may ask you to provide additional information, such as your insurance information or a list of your medications.

3. You are contacted:

  • The referral service will contact you to schedule an appointment or to provide you with information about their services.
  • They may also provide you with resources or information that you can use while you are waiting for your appointment.

4. You receive services:

  • You will receive the services you need from the referral service.
  • The referral service will work with you to develop a plan of care that meets your individual needs.

5. Follow-up:

  • The referral service may follow up with you after you have received services to make sure you are satisfied with the care you received.
  • They may also ask you for feedback on your experience with the service.

NOTICE:

  • The amount of time it takes to receive services will vary depending on the service and your individual needs. As a GP, we can not urge the referral service’s to see you, as soon as possible.

Social Support Service’s

Support services are like a safety net for people who are going through a tough time. They can help with all sorts of things, like mental health, physical health, finances, and relationships.

Falls prevention

This service helps people over 65 who are at risk of falling. If you’re eligible, you can refer yourself directly to the service, by emailing them.

Wheelchair services

This service provides wheelchairs to people who need them. You can refer yourself to the service, or you can be referred by your GP.

You can self-refer to our sexual health and contraception services for STI testing, treatment, or advice on contraception. Sleep Station is an online program that can help. It uses CBT, a type of therapy that can help you change your thoughts and behaviours.

Brent Services Podiatry Centre – Contact Details

All enquiries and appointment bookings for the podiatry service are handled by the administrative office at the Brent Services Podiatry Centre. The office is open Monday to Friday, from 8:30 am to 5:00 pm.

For assistance, please call 020 8937 6365. An answering machine is available to leave messages, and these are checked daily. Be sure to speak clearly and provide a contact number for a return call

You can self-refer to the Brent Integrated Musculoskeletal Service (IMS) for physiotherapy by either calling 020 3795 3429 or using the online self-referral button.

PhysioNow is a digital assessment tool that guides you through a series of questions to help direct you to the most suitable treatment for your pain or injury. Completing the assessment allows a qualified healthcare professional from Connect Health to review your information and determine the care you need, ensuring you get the right support faster. We highly recommend using this service.

Children Service’s

Children’s service’s are a support system for kids and families, who are struggling with their daily routines. They can help if a child is having trouble at home or school, or if they’re not feeling safe or well. They can also help families who need a little extra support.

Weight Management Service (Tier 2) – Children

Brent Council has partnered with BeeZee Bodies to provide support for children and families in Brent who want to live a healthy lifestyle.
