How to arrange a GP Consultation

Patchs

We use a range of methods to provide consultations and medical care with our doctors and other clinicians.  However, we ask all patients wanting medical advice or care to first complete a Patchs online form, and this will allow our doctors to review your request for help and decide on the best means to offer care. If you are unable to use PATCHS ONLINE we now use a PATCHS voice recognition system to make it easier to contact our team. The PATCHS Telephone Assistant allows you to telephone us without the hassle of waiting in long phone queues. You can leave a voice message detailing your request, without needing to wait to speak to a receptionist.

The PATCHS form (either sent Online or via Patchs Telephone Assist) will be reviewed by the GP within 36 hrs and you will then be contacted by a member of our team by phone or text with a plan for the most appropriate next steps.  You will be called or texted by a doctor, nurse or receptionist, in order to make a plan with you to deal with your medical needs.

You may be texted or called by our doctors directly to deal with the problem, or instead asked to book a face-to-face appointment with a GP, or asked to have blood or other tests before seeing or speaking with a clinician. 

Our Advanced Nurse Practitioner is an advanced registered nurse and prescriber.  Jevan is trained to assess patient needs, order and interpret diagnostic and laboratory tests, diagnose disease, formulate and prescribe medications and provide treatment plans.

Extended Hours Appointments

Midweek GP Appointments:

We now offer extended hours GP appointments on some mid-week evenings between 6.30-8pm

(the day may change from week to week).  These appointments may be face to face or telephone consultations and are bookable through the reception team.

Saturday Appointments:

We now offer extended hours GP, Nurse and HCA appointments on Saturdays between 9am-5pm.

These appointments are offered within our Primary Care Network (PCN)  This is a group of 5 local surgeries within Hammersmith and Fulham that we collaborate closely with to ensure the best care for all our patients.

The Saturday appointments are offered on a rotation system between the 5 surgeries so will not always be here at Milson Road Health Centre.  This will be explained to you by a member of the reception team when booking.  A limited number of the Saturday GP appointments are bookable online.  If you book one of these appointments online please check the confirmation text carefully to ensure you attend at the right surgery

Cancellations

If you cannot attend an appointment for any reason please inform us as soon as possible in order for us to give the slot to someone else.

Patients who miss three appointments without letting us know may be removed from the practice list.
