The fees charged are based on the British Medical Association (BMA) suggested scales and our reception staff will be happy to advise you about them along with appointment availability.

| Letter request – To whom it may concern | £30.00 | Any letters addressed to a third party relating to patients. This includes school letters, blues badge requests, holiday letters etc. |
| Form to be completed | £30.00 | Any type of form requesting patient information and GP completion. This includes; patient insurance forms, student/university reports, work/employment reports, fit to travel forms. |
| Adoption Medicals | £100.00 (per parent, including medical assessment) | Adoption medical requests for assessing patients for adoption. This includes report completion + medical assessment. |
| Capacity Assessment (1) | £60.00 | Assessment for capacity + report completion for private or medical reasons. |
| Capacity Assessment (2) – if home examination is required | £120.00 | Assessment of capacity INCLUDING home visit. This includes full report completion + home visit. |
| Driver Medical Examination | £100.00 | Medical examination for drivers including appointment and report completion. Payment is to be made to GP directly during appointment, and digital receipt can be requested if required. This fee applies for; driving license category renewals, PHV/Private Hire Medicals, New license/applications following revoked licenses. |
| Private Healthcare Report Request | £50.00 | Medical report requests for the purpose of seeking private healthcare + insurance cover of healthcare. This includes full report completion + telephone appointment if necessary. These can come from companies such as; Bupa Health, Vitality Life, Legal & General, Nuffield Health etc. |
| Power of Attorney Reports | £80.00 £150.00 (if home visit required) | Power of attorney assessment + report completion included in fee. |
| Verification of documents | £30.00 | Some GPs are happy to confirm validity of documents/sign as witness. This request will need to be discussed with a GP before payment is made to confirm GP is happy to complete. |
