How to Register
The Abingdon Medical Practice welcomes new patients.
Adults:
In order to register please fill out the PRF1 form and Patient Registration Questionnaire (please see below) and email them to abingdonmedical@nhs.net. Or, alternatively, you can register online here.
Children:
For children please fill out the PRF1 form and A Patient under 5 years old Registration Questionnaire and email them along with a copy of the immunisation record to abingdonmedical@nhs.net. Or, alternatively, you can register your child online here. Please remember to send a copy of the immunisation record to abingdonmedical@nhs.net.
Catchment Area
To register at the practice you need to live within the practice catchment area (see below). If you have moved into the area from another GP practice you need to provide your previous GP plus your NHS number. This helps the NHS arrange for the transfer of your medical records from your previous practice.
You will be contacted by a staff member in order to book a telephone call with our Nursing Associate or a Practice Nurse who will complete the registration process.

- A Patient Registration Questionnaire
- A Patient under 5 years old Registration Questionnaire

We recommend that you see the same GP on an on-going basis. This may mean you have to wait a little longer to get an appointment with your doctor who knows your history. Please always let reception know who your usual doctor is when booking an appointment.
