Receptionists provide an important link for patients with the practice and are your initial contact point for general enquiries. They can provide basic information on services and results and direct you to the right person depending on your health issue or query. Receptionists make most of the patient appointments with the GPs. They also perform other important tasks such as issuing repeat prescriptions and dealing with prescription enquiries, dealing with patient registrations and records and carrying out searches and practice audits.
MCMW – My Care My Way
Hodan Yusuf, the Senior Case Manager for West London Planned Care, leads on home visits with Dr Hibbert; looking after a significant number of our patients every week who have complex health and social needs. We know this service is invaluable to many of our more elderly and vulnerable patients.
Social Prescriber
Tom Newland – Tom looks after our patients (below 65 year old) who need community-based support. Tom can help patients who would benefit from community services that offer practical, social, and emotional support for health and wellbeing. This includes connecting people to statutory services for example housing, financial and welfare advice.
Teaching
This is a teaching practice. Doctors and medical students studying general practice may be working with your doctor occasionally.
