Appointments for Travel advice/vaccination
If you require any vaccinations relating to foreign travel, please complete the Travel Questionnaire – Online Form. Once submitted, please ring the surgery on 020 8529 5543 to book an appointment with the Practice Nurse.
It is important that you submit the form and make this initial appointment as early as possible – at least 6 weeks before you travel.
During the consultation, the Nurse will discuss your travel arrangements to determine which countries (and areas within countries) that you are visiting and the travel vaccinations required.
Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS. The Reception team will advise you if there is likely to be a charge before you attend your appointment.
