Making a referral
It’s quick and easy to refer a young person to one of our community teams. Just contact the team that’s convenient for them. You can find their contact details on the map above.
Please provide as much detail as you know about the needs and experiences of the young person and make sure you seek their informed consent before making a referral.
Young people can also refer themselves if they prefer.
How it works
Once we receive a referral, we will contact the young person within a few days for an initial appointment where we can chat through what difficulties they’ve been having.
From there, we can start working on an action plan to address those challenges together and talk about the types of support available so they can decide on what they feel might be best.
Once the programme of support starts, we monitor progress through regular meetings and reviews with the young person at a convenient time and way for them.
Outcomes
Young people benefit in several ways from our service including:
- More resilience and confidence to face life’s challenges, reducing the need for ongoing support from adult mental health or crisis services
- Improved achievements and confidence at school or work
- Better, more positive social and emotional relationships with others
- Improved self-esteem and aspirations
Consent
Please ensure that you have the consent of the young person you are referring to do so. As we are funded by Conexus Healthcare, all information is handled and processed in line with their policies. Please note that the link will take you away from this website.
