New Patient Registration

You will be asked to complete a health questionnaire when you first register with us. This questionnaire is important as it provides us with background information about you.

Practice Area

To register at the practice you need to live within the boundary area. Please enter your postcode below to check whether you are.

Catchment Area

We only cover parts of:-

· Harrietsham

· Bicknor

· Otham

· South Green

If you live in these areas and it says you are just out of the boundary, then please give us a call and we can double check.

How to Register

We have an open list and welcome requests for registration from patients living in or moving to the practice area.

You can register by completing a form available from reception or by clicking the link below. When you register, you will be asked to provide 2 forms of identification. One of these should have your address in our area on it, the other should be a passport, driving licence or birth certificate. It takes 7-10 days for your registration to be complete.

Non-urgent advice: Notice

PLEASE MAKE SURE YOU REGISTER WITH A DOCTOR AS SOON AS YOU MOVE INTO THE AREA. DO NOT WAIT UNTIL YOU NEED TO SEE A DOCTOR!

From 2nd October, the new registration form will include supplementary questions for overseas visitor patients to declare when they have a non-UK European Health Insurance Card (EHIC), Provisional Replacement Certificate (PRC) or S1 form.

These questions will be used to determine an overseas patient’s eligibility to free NHS Secondary healthcare. All new patients will be given a leaflet to explain this process.
