Register to use online services

To use online health services, you need to register and prove who you are. There are 2 ways to do this.

Prove who you are online

Once you have created an NHS account it will ask you to prove your identity.

To prove who you are you need to:

  1. Take a photo of your ID (passport, UK or EU driving licence, or European national identity card) using your phone.
  2. Record a short video of your face while you say 4 numbers, or use your camera to scan your face.

Prove who you are through your GP surgery

You’ll need to:

  1. Fill out the online registration form.
  2. You will be asked to upload proof of ID and address.
  3. Once approved, you will be given 3 registration details. This is not the same as email address and password you use to log into your GP surgery’s online services.

The 3 registration details are:

  • Linkage Key (could be called Passphrase)
  • ODS Code (could be called Organisation Code or Practice ID)
  • Account ID

If you are unable to fill out an online form, you can come into one of our surgeries and fill out a paper copy at reception. Please remember to bring ID with you.


Accessing GP services for someone else, with proxy access

GP surgeries can give someone secure access to another patient’s GP services, so they can help them manage their health and care.

This is sometimes called proxy access or third party access.

You can ask to be a proxy, to help someone else with their prescriptions, appointments or other GP services, or choose a proxy to help you with yours.

Setting up proxy access

  1. Fill out the online registration form.
  2. You will be asked to upload proof of ID and address.
  3. Once approved, you will be given 3 registration details. This is not the same as email address and password you use to log into your GP surgery’s online services.

  • ODS Code (could be called Organisation Code or Practice ID)
  • Account ID

Parent and guardian proxy access for children under 16

Information:

You can only get online access to the GP services of a child aged under 16 if you are their parent or guardian with legal parental responsibility.
