Please complete an Accurx Triage request if a Sick Note is needed, the first 7 days you are off you can self-certify, weekend days are included in the count of days.
If a further certificate is need following the initial issues, please complete another Accurx Triage request, the additional Sick Certificate cannot be completed in advance of the date needed.
