Record Sharing
Record sharing allows health and social care staff outside your GP surgery to have access to your health information, approx 98% of patients already have a summary care record (SCR) which shares medications and allergies. In order to provide access to more detailed information which may be useful to services such as emergency departments, then you must consent to SCR with additional information. Please read the leaflet below for more info.
Please be aware that for patients over 16 with capacity we are not able to discuss treatment or management of the patient with family members/guardians unless the patient is also present.
Consent from the patient will allow us to disclose information already on file such as test results, or discuss matters such as arranging appointments or hospital transport with their nominated representative.
For patients lacking capacity. In order to release information or to be able to discuss treatments with family members or representatives we must see evidence of an existing lasting power of attorney or an order under the court of protection.
