We welcome new patients who live within our practice area.
Pre-Registration With the Practice
We will now be requesting presentation of 1 forms of ID for all newly registered adult patients, providing it has both a photo and a proof of address eg driving licence which holds the correct address you are registering with. If not, you will need to provide both photo ID and proof of address in the form of a utility bill, council tax letter, bank statement within 3 months of issue will be accepted.
IF no photo ID is available a birth certificate/marriage certificate or similar will be acceptable.
You can come to the practice to fill in a registration form, new patient questionnaire and consent forms for record sharing and online access. Or use the links below to print and complete these forms at home.
When registering with The GROVE MEDICAL CENTRE can patient fill in the registration form, the correct new patient questionnaire adult/child, and the SCR form bringing all paperwork with them with photo ID and proof of address.
Online Access- Needs 2 forms of ID, one must be photo ID please.
Proxy access for parents as from April 2020 is granted (with evidence of parental responsibility) until the child reaches 11 years old. Access will then automatically cease. Between the ages of 11- 16 years old, this access will NOT be reinstated.
ADULT (PRINT ALL 3 FORMS) – For all patients over the age of 14 years old.
