Non-urgent advice: Please be aware
Your registration will be processed in about three weeks. In the meantime, if you need medical help, you can contact your previous surgery for advice or visit the Urgent Treatment Centre at Crawley Hospital.
When registering:
- Everyone is welcome
- Once the registration has been processed you will receive a letter of confirmation. We will also be offering a New Patient Screening.
- You do not need to provide proof of ID, address, immigration status or an NHS number in order to receive care or see a GP, but it might help.
New Patient Registration Forms
Named GP
All patients at Bridge Medical Centre have a named, accountable doctor who is responsible for coordinating their care. Your named doctor will be allocated to you by the Practice.
You can still talk to or make appointments to see any of our doctors or nurses, not just your named GP
Find Your NHS Number
Sometimes when filling in online forms or speaking with our staff, you may be asked for your NHS number.
This online tool will help you find your number if you do not know it.
Other ways to get your NHS number
If you cannot get your NHS number online you can:
- find it on any letter from the NHS like a prescription or appointment letter
- call your GP surgery and ask them for your number

You do not need to know your NHS number to use NHS services, but it can be useful to have it.
