Supporting documentation accepted:

Photographic Identification for Adults i.e. Drivers Licence, Passport
Birth Certificate for Children
Proof of Address i.e. Utility Bill – This is to help us match you to any pre-existing NHS records

Cornerways Surgery Patient Information

Children (<18) & Adults who need help making decisions

Children under 16 need to be registered by their parent or guardian. A separate form needs to be completed. You may also be asked to provide a form of ID to prove that you’re their parent or guardian.

If you have a personal child health record (red book) from the birth of your child, it will help to provide this. If your child received any vaccination outside the UK, please provide supporting documentation.

For adults who cannot make their own healthcare decisions (lack capacity), a relative, primary carer, lasting power of attorney or court appointed deputy may apply on their behalf.

Non-urgent advice: Patient notice

We aim to complete registration within a fortnight and invite you to a new patient registration telephone appointment. If you have a more urgent need, let us know.
