Registering as a patient at Testvale Surgery
We are using a new online service called Register with a GP surgery that makes it easy to register with Testvale.
Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.
The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.
Paper forms are still available if you need one.
In order to register fully, all patients must:
- Live within our Practice Catchment Area – please see the map above.
- Anyone regardless of their country of residence or immigration status is entitled to receive free NHS primary medical services at a GP practice. Please be aware that hospital services may be chargeable.
New patients are registered equally among the GPs but you have the right to state a preference about which doctor you see. To make sure that you get the best continuity of care it is preferable to keep to one doctor as far as possible.
New patients may be invited to see the nurse for a registration health check, which includes various physical checks, including having your blood pressure measured, and a discussion of your answers on the questionnaire. You may be required to provide a urine sample.
If you are a new patient who has ongoing medical problems or who takes any kind of medication, you should book an appointment with your doctor to discuss your ongoing medical needs before requesting any ‘repeat’ prescriptions from us – we would like to meet you.
Most of our consulting rooms are on the ground floor and there is a lift to the first floor. Our staff are always happy to help patients who are having difficulties.
