If you would like to register as a patient, please follow these steps:

1. Check you live within our practice boundary

To register as a patient with us, you will need to live within our practice boundary. Use this catchment area map to check if you live within our boundary.

  • You can use your postcode to search.
  • If you are unsure, contact us.

2. Complete a registration form online

You can also download and print a paper form to fill in by hand.

Temporary Patient Registrations

If you are not registered with a doctor but need to see one, you can register as a temporary patient. Please ask for a paper form at one of the practice receptions.

Find your NHS Number

You do not need to know your NHS number to use NHS services, However, it can be useful to have it.

This online tool will help you find your number if you do not know it.

Other ways to get your NHS number

If you cannot obtain your NHS number online you can:

  • find it on letters or forms from the NHS, e.g.: a prescription or appointment letter
  • or contact us to request your NHS number.
