Register as New Patient

You can register as a patient with the Practice by completing a patient registration form (GMS1) below and handing to reception.

You will also find new patient questionnaires which you will need to complete for each member of your family wishing to join the Practice; there is a form for children (under 13) and a different one for older children and adults, each of these forms can be dowloaded via the links below.

Identification

We will also ask to see two forms of identification;

• A photo ID (if you have it) and;
• A proof of address, such as a gas or electricity bill (no more than 3 months old and at your current address)

If you have no photo ID, you will need 2 of these documents or your birth certificate. 

Prescriptions

All new patients will be requested to nominate a pharmacy for electronic prescription services.

If you are currently on prescribed medication you will need to make an appointment to see a GP before we can issue you with a new prescription.

GP of Choice

Patients have the right to express a preference to receive the service of a particular doctor. If you wish to do this, you must let us know this in writing.

Whilst the Practice will endeavour to comply, it might not always be possible to do so.

Temporary Patient Registrations

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

Disabled Patient Facilities

There is access through the main door. We have a wheelchair available for use in surgery.

Outside the Practice are disabled parking bays and mobility scooter parking.

Hearing Difficulties

If you are experiencing hearing difficulties when being called in to see the doctor or nurse, please do let us know in order for us to set up an alert on your medical records and personally collect you from the waiting room. Alternatively, we do have the facility of a portable induction loop. If you would like to use this, please ask at reception for assistance.
