If you have received a text message from East Berkshire Primary Care about Remote Monitoring, it means we believe this clinically led service may benefit you. The message will include details about the service and inform you that a Digital Health Team member will be in touch to discuss it further.


How Remote Monitoring Works
The service enables you to regularly share your health readings, such as symptoms and blood pressure, with a Digital Health Team. These healthcare professionals can monitor any changes, offer clinical advice, and provide further support when needed. Previously, you had to contact your care professional when you needed help. Now, healthcare professionals can proactively support you by combining their clinical expertise with information from your shared care record and remote monitoring data. This means you can receive timely assistance, reducing the likelihood of emergency treatment.


How We Use Your Information
Your health data is securely collected and monitored to support your care. Only relevant healthcare professionals will have access to your information. All data is stored securely following NHS confidentiality and data protection policies. We will not share your data with third parties outside your care team without your consent unless required by law.


What Happens Next?
If you agree to be enrolled onto remote monitoring, a member of the Digital Health Team will contact you soon to discuss the service and any equipment you may need. You will also receive guidance on how to set up the app to begin sharing your health readings.
