District Nurses
District nurses care for patients who can’t visit the practice and need skilled nursing care at home. These patients might be recovering from a major operation, or be elderly and too frail to care for themselves. If you are housebound, or you care for someone who is, our nurses can give you plenty of information, advice and support.
If you or someone you care for might need the help of a district nurse, you can call on 01865 903750.
Health Visitor
The Health Visitor is a nurse who has been trained in child health and development. Health Visitors are here to support families and their babies, from the birth until the child starts school.
The Health Visiting Service is run by Oxford Health NHS Trust and looks after families with children under 5 years of age.
The Health Visitors are based in Wallingford but cover Clifton Hampden and Berinsfield surgeries. More information can be found on their website https://www.oxfordhealth.nhs.uk/health-visitors/ or you can find them on Facebook @OxfordshireHealthVisitors
If you need to contact a Health Visitor for support or advice you can text the Health Visiting message service ParentLine 0-4 years on 07312 263081.
You can see the health visitor by appointment only (available 09.00am to 16.30 noon, Monday – Friday), or by visiting the clinic on a Monday at The Berin Centre (Berinsfield) by appointment only (13.15pm – 14.45). The team can also arrange a home visit if necessary.

Social Prescriber
Did you know that you can improve your health through social prescription?
Social prescription will link you to non-medical support and there is good evidence that this can lead to positive health and wellbeing outcomes.
- You can be referred to a social prescriber by any of the health care staff at Clifton Hampden Surgery.
- The social prescribers will be able to give you more time to focus on what matters to you.
- They can connect you to community groups and agencies for practical and emotional support; helping you to access local clubs, activities, education, leisure facilities, recovery projects, voluntary organisations and more.
- Becoming connected and engaged in your local community can help you feel supported and can help you feel better.
If you feel that this service would be of benefit to you, please mention this at your next appointment.
Your local social prescribers are Ann Mitchell and Claire Carrera.
NHS England and Improvement (NHSE/I) East of England team have produced a series of useful animations to introduce the extended healthcare team to the patients, carers and service users:
