New Patient Registration

We are now able to accept online registrations.

Clifton Hampden Surgery is accepting new patients. Anyone in England can register with a GP surgery. You do not need proof of address or immigration status, ID or an NHS number, however we do ask for this information when you register if you have it to help us ensure all of your key information is correctly entered. More information about registering with a GP surgery can be found here.

• If you have confirmed you live inside the above practice boundary, you can come to the surgery to collect a registration pack, or download them below.

• If you are registering a child under 5, please also completed the Child Immunisation History Form – download here

If you wish to register for online patient access, please read the information and complete the online access registration form which can be found in the Health Profile download above. We can then use your identity documents presented at registration to verify your identity for online access.

To complete your registration, please ensure you bring the following documents to reception:

• One of the following proof of address:
• UK bank/building society statement issued within past 3 months
• UK credit card statement issued within the past 3 month
• Council tax statement issued within past 12 months
• Valid work permit/visa (UK)
• Benefit statement eg pension
• Sponsorship letter from your employer/future employer (for non-UK, non- EAA applicants only)

Completed registration pack (all enclosed forms)
INCLUDING THE PURPLE GMS1 FORM.

• One of the following identity documents:
• Valid passport
• Valid driving licence
• Biometric residence permit
• Valid EU national ID card
• Original birth certificate (this is all that is required if registering a child)
