We are using a new online service called Register with a GP Surgery. That makes it easy to register with Old School Surgery.

You do not need proof of address or immigration status, ID or an NHS number.

Although UWE students can complete a separate registration form for Uwe Health Centre, as Old School Surgery is one practice, you may be asked to attend either site.

The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

All registrations must include BOTH forms below.

• Form 1 -NHS GP registration Form 
• Form 2 -Old School Surgery Registration and Health Information Form

We ask you to complete the health questionnaire at the time of registration. This questionnaire will include questions about lifestyle and medical history and must be completed alongside the NHS registration form in order to register.

If you wish to gain online access to your medical records, we will need to see you in person with some photo ID to verify your identity. This is due to the confidential information accessible on the online access platforms. 

You can hand in completed registration forms at your local surgery branch.

You can only Register with us if you live within the Old School Surgery Boundary. To find out whether your address falls within this area please see the map below or alternatively the admin team can be contacted via email on bnssg.general.oldschool1@nhs.net.

If you are not registered with a doctor but need to see one, you can receive emergency treatment for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

If you are moving house, please let us know as soon as possible. If you are moving out of the surgery’s catchment area, you may be asked to register with a surgery that’s closer to your new home address.
