How To Register
If you wish to register as a patient at our practice your will need to complete our registration forms.
You can do this by;
- Registering online
- By visiting our practice or branch surgery and collecting the registration forms
- By downloading and completing the following forms: GMS1 and either Children Under 15 Welcome Pack or Adults 15 Plus Welcome Pack
Whichever method you choose you will need ID and proof of address on to complete the registration.
If you need assistance one of our Patient Services Assistants Team will be happy to talk you through the process.
The medical questionnaire is required because it can take a considerable time for us to receive your medical records.
Allocation To A Named Doctor
If your registration is successful you will be notified in writing of your allocation to a named GP.
Patients have the right to express a preference to receive services from a particular doctor or professional group and the practice will endeavour to comply with any reasonable preference expressed.
Nevertheless, the practice has the right to refuse such a request where it has reasonable grounds to do so; this includes equity of distribution of patients across GPs.
Overseas Visitors
If you are visiting from overseas you may not be entitled to free treatment, it partly depends on whether your home country has reciprocal arrangements with the UK.
