New Patients

How to register as a patient

If you are new to the area, live within our catchment area (please see map below) and wish to register as a patient, please ask at reception for a registration form for each household member. Alternatively you can print the forms by clicking the links below. Once you have completed the forms, please hand them in at reception.

You will be registered with New Road Surgery and not a specific doctor (despite one of their names appearing on your new Medical card when you receive it). Any of the doctors will be pleased to see you, or you may prefer to request a specific doctor; please ask reception.

We will endeavour to accommodate your preferences but wish to remind you that this may not always be possible.

New Patient Information

Non-urgent advice: Polite Notice

At New Road Surgery, we make every effort to provide as many appointment slots as possible. Recently we have noticed an increase in patients failing to cancel appointments they no longer need. This results in wasted appointments and patients having to wait longer to see a doctor.

If you make an appointment and find that you cannot attend, it is essential that you contact the practice to let us know. This helps everyone to be able to see a doctor more quickly. it is practice policy to consider removing patients from our list if they fail to attend to cancel appointments.

Thank you.
If you are local and are under the care of Longbridge Mental Health Team, please be aware that if you join this surgery, your care will have to transfer to Worcestershire Mental Health Services.
Newly registered patients aged five and over are invited to attend the surgery for a health check when joining the practice. This includes a brief look at your medical history, details of any medication and advice about health including weight, diet, exercise and blood pressure. Please book your appointment at reception. Please bring a urine sample with you to this appointment.

New Patient Health Questionnaire – we would be grateful if you could complete this form to enable us to update our records.
From 1st April 2015, the practice is required under the terms of their GP contract to allocate a named accountable GP to all patients (including children). The named GP will be responsible for the patients’ overall care at the practice. It is important to note that you will still be able to make your appointment with any GP at the practice.

For new patients, your named GP will be allocated as follows:
– Surname A-M: Dr Kotekar
– Surname N-Z: Dr Hussain

Having a named GP does not prevent you seeing any other doctor in the practice.

You do not need to take any further action, but if you have any questions, or wish to discuss this further with us, please ask reception when you are next at the surgery. if you have a preference as to which GP you are assigned, we will do our best to accommodate your request.
We have a Practice Privacy Statement available in the waiting area and on our website (under Practice Policies).

Text messages: We use your mobile phone details to unable us to send text messages to notify patients aged 16 of forthcoming appointments. We use your mobile phone details to enable us to send text messages to notify clinical services such as flu clinics and occasionally to offer additional services such as NHS Health Checks. If you are not happy for u to use your data to send you the above types of information, please advise reception.

It is important that your update your mobile number with us if it changes, so that we send your information to the correct number.

If we don’t have your mobile number and you want to receive the above messages, please let us know.

When you register you will also be asked to fill out a New Patient Medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you should complete and return to us with the New Patient Registration Form.
