New Patient Registration

The doctors welcome new patients who live within our practice area. 

Registration can be achieved on completion of the appropriate forms, which are available from Reception. You will be asked to produce evidence of identity and residency, e.g. a passport and household bill. We are unable to register patients (including temporary residents) beyond our practice area.

Registration forms can be completed online but, we still require you to come in to the surgery to present two forms of identification listed above. We also require a signature from the registering patient where appropriate. Until identification has been produced the registration form will not be completed.

Registering with the practice

When contacting the surgery for the first time please mention that you have completed these forms online. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct – please note we are only able to register you once you have signed the forms and produced your identification. You also need to fill out a medical questionnaire when you register. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
