Urgent appointments
If you feel that you have a medical problem that requires urgent assessment you will be referred to the Urgent CareTeam. The team consists of a Duty Doctor, an Advanced Nurse Practitioner and a Primary Care Practitioner. The team work together to assess your problem and organise a pathway of care that is the most appropriate for you.
Please telephone the surgery on 01989 763535 as early as possible, and state that you need to be seen urgently. The reception staff will ask a few questions to help assess your condition. You will then be offered either a telephone appointment or asked to come in to the Urgent Care Team Clinic.
Routine appointments
To request a routine appointment in the next 14 days:
- use your NHS account to book an appointment
- Use our contact us form on our Home page
- phone us on 01989 763535, Monday to Friday from 8 am to 6.30 pm
- visit the surgery and speak with a receptionist, Monday to Friday from 8 am to 6 pm
When you get in touch, we will ask what you need help with.
Our Care Navigation Trained receptionists will use the information you give us to choose the most suitable doctor, nurse or health professional to help you.

However you choose to contact us, we may offer you a consultation:
- by phone
- face to face at the surgery
- on a video call
- by text or email
Appointments by phone, video call or by text or email can be more flexible and often means you get help sooner.
Cancelling or changing an appointment
To cancel your appointment:
- use your NHS account
- phone us on 01989 763535 Monday to Friday from 8am to 6.00pm
- reply CANCEL to your appointment reminder text message
If you need help when we are closed
If you need medical help now, use NHS 111 online or call 111.
NHS 111 online is for people aged 5 and over. Call 111 if you need help for a child under 5.
Call 999 in a medical or mental health emergency. This is when someone is seriously ill or injured and their life is at risk.
If you need help with your appointment
Please tell us:
- if there’s a specific doctor, nurse or other health professional you would prefer to respond
- if you would prefer to consult with the doctor or nurse by phone or face-to-face.
- if you need an interpreter
- if you have any other access or communication needs
Home visits
If you are registered as housebound and need an appointment, we will phone you first to understand what you need.
Requests to the surgery for a home visit by a clinician need to be made to the practice as soon after 8 am as possible and preferably before 10.30 am to ensure enough time for the clinician to make arrangements.
Hereford General Practice Extended Hours Service
Hereford General Practice Service offer numerous appointments with S&W PCN (Primary Care Network). For the extended GP service, please call the Surgery to ask for an appointment or telephone 111 and ask to book an appointment with the Hereford General Practice Extended Hours Service.
You can also call NHS 111 at the weekend or Bank Holidays.

Benefits to staff and patients
Saves money
The NHS spends millions of pounds on SMS costs each year. We’re making NHS App messaging available to NHS commissioned services for free.
Safer and more secure
Messaging via the NHS App is more secure than other channels like SMS. SMS can be vulnerable to interception and spoofing but sending your patients messages to their NHS App secure inbox is much more secure. For patients, this means they will always know who messages come from, rather than seeing a messages from unidentified mobile numbers.
More reliable
NHS App messaging means even if a patient changes their contact details, you will still be able to contact them.
Better content
When sending messages via SMS, the length is limited. NHS App messages can be longer and richer in content, for example with branding, link texts and interactive elements.
