🏃🏼The Healthy Lifestyle Trainer Service🏃🏼♀️➡️
🗣️ The HLTS offer free and confidential support to anyone aged over 16, living in Herefordshire or registered with a Herefordshire GP.
ℹ️ They can support people to make healthier changes to their lifestyle, particularly focusing on:
🍓🥦Healthy eating – to include meal planning, cooking tips & ideas, food budgeting.
🍎🏊🏻Weight management – client led, personalised plan, supporting clients to set simple goals towards achieving a healthy weight and increasing motivation and confidence in change.
🤸♂️⛹🏻♂️Increasing Physical activity – supporting clients to set a personalised plan around increasing their activity levels, supporting them to access physical activity sessions within their local area.
❌ 🚬 Stopping smoking – with access to Nicotine Replacement Therapy (NRT) and/or 8 weeks’ worth of vape kit.
❌ 🍷Reducing alcohol – setting a personalised plan to make swaps and changes to reduce their alcohol intake to recommended levels.
👥The HLTS can also help people identify how their behaviours in these areas may be affecting their health
🧎🏾♀️➡️This is achieved through a behavioural change approach, supporting individuals to create a personal health plan to make changes to improve their health and wellbeing. The HLTS also signpost to other agencies and professionals depending on the needs of the client.
🫂This can be done via 1-1 support or small group sessions, the health and wellbeing trainers can provide up to 12 weeks of support, over 6-8 sessions, delivered within the local community.
