Form to fill in for Urine infections

Cystitis or Urine infections and their management (Information for patients)
We have changed the way we manage urine infections at the practice. If you think you have a urine infection you will be asked to fill in the form below and a clinician will call you back on the same day.
PLEASE DO NOT HAND IN A URINE SAMPLE UNLESS YOU GET ASKED TO DO SO.

Procedure for Sick Notes

Sick notes can now be requested upto 2 weeks prior to the expiry of a previous sick note.

We will usually be able to provide a sick note within 5-7 working days of receiving your request so please request your sick note with plenty of time so that you do not run out. In order to obtain a sick note all patients must fill in the following form below.

Practice Counsellors

You can book a telephone appointment with our counsellors Siri or Rosie. There is no need to see a doctor or clinician beforehand. Simply call reception and ask for an appointment with a counsellor. They can offer you telephone counselling or put you on the waiting list for Face to Face Counselling with our Student Counsellors Nadiya, Pete and Rachael.
