We are using a new online service called Register with a GP surgery that makes it easy to register with Portcullis Surgery.
Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.
The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

You can also download the forms in the links below and print them off at home or collect a paper copy of the form from the surgery. If you have difficulty filling in forms please ask a member of staff to help you fill in these forms.

Out of Area Registration
We will consider registration of patients outside our catchment area for any address within Shropshire and Telford, ideally within 20 miles of the practice. This would be registered as an out of area registration, the proviso is that we will not carry out visits to your home and you must be willing to travel to the practice if needed for your medical care.
